The AFL-CIO and “My Health Solutions” is sponsoring a flu and COVID-19 vaccination drive-thru service on Saturday, October 2, 2021 from 8 AM-12 PM.
It will be a on a first-come, first-serve basis and you must wear a mask and bring the following items with you (see below for forms):
-Completed flu and/or COVID-19 Vaccination Questionnaire and Consent Form
-Photocopy of your Government-issued ID and medical insurance (if you do have insurance)
-Minors under 18 must be accompanied by a parent or legal guardian
-COVID-19 shot record (if you have received previous doses of Pfizer or Moderna)
REQUIREMENTS:
3 years or older for flu shots
12 yrs-older for Pfizer-BioNTech
18 yrs-older for Moderna
18 yrs-older for Janssen (Johnson & Johnson)
